Would this patch help with sciatica or pinched disc pain?

It is a very temporary way of getting relief. I use it for cervical dystonia. We have always used the roll on version for general arthritis/muscle pain. In all honesty, I get just as much relief from Icy Hot patches and I can purchase them locally. I'd give them a try. They do adhere well. Good luck.

Do these last longer than the gel? I've been using the gel but the effect wears off too quickly and I have to frequently reapply.

I had tried the gel, the patch and the spray. Both the gel and pad eventually wear off like any other topical pain med but personally I was surprised by the spray. Seemed to last the longest and was no mess, easy application. Just spray and go. I definitely would reach for the spray depending on how much pain I was in.
